STEP ONE

Complete the mental health or medical provider referral form.

STEP TWO

Send your clients the links to the intake forms that best suit them:

If your client is a child or adolescent, send them the links for  "Caregiver Intake Form"  (appropriate age), "School/Teacher Intake Form."  Ask the parents to send the teachers the link.

If your client is 12 years and older and can read and write, send them the links for the "Caregiver Intake Form" (appropriate age), "Adolescent Intake Form" and "School/teacher Intake Form." Ask the parents to send the teachers the link.

If your client is 18 years old or over, send them the "Adult Client Intake Form"
